<p>Patients affected by vitiligo have a predisposition to develop any related diseases such as autoimmune Addison&#8217;s disease, thyroid problems, diabetes and alopecia areata (loss of scalp hair in patches). Many of these ailments can be diagnosed by blood tests or a simple physical examination.</p>
<p><strong>What is the treatment to be followed? </strong></p>
<p>Patients must use effective sunscreen to avoid burns in the affected areas of skin. There are several treatments the aim is to hide or camouflage the discoloration, or prevent progress. None of these are foolproof and all have their limitations.<span id="more-718"></span></p>
<p>Camouflage: depigmented areas are covered with stains or dyes topics. In the medical makeup can find useful tips and information.</p>
<p>Photochemotherapy, has frequently been used for vitiligo. This treatment combines a drug (psoralen) activated by a light bulb with ultraviolet A (UVA). Studies show that PUVA treatment offers only a slight improvement over placebo (dummy treatment).</p>
<p>Corticosteroids for vitiligo apply both topical (applied to the skin) and systemic (usually oral). Although there is no doubt that the degree of improvement is higher compared to placebo therapy, less than 50% of patients respond to this treatment.</p>
<p>Skin grafting: in patients with very dark skin, the graft pieces of pigmented skin from other parts of the body, can help or induce repigmentation of affected areas.</p>
<p>Depigmentation: very large depigmented areas can be applied in a cream substance, such as hydroquinone, which depigmented areas of skin are normal. Sometimes the best approach is for the patient.</p>
<p><strong>What is the prognosis? </strong></p>
<p>Vitiligo usually progresses slowly. Although spontaneous resolution is rare, partial remission is possible in the months that there is more exposure to the sun. The treatments are generally unsatisfactory. Various makeup products offer a cosmetic solution to the affected patients.</p>

<p><strong>What causes vitiligo? </strong></p>
<p>Normal skin contains melanin, a brown pigment produced by the tyrosine skin pigment cells (melanocytes). If you look under a microscope the skin affected by vitiligo, we can see that there is an absence of melanocytes and that the deepest layer of the skin shows signs of inflammation.<span id="more-714"></span>The exact causes are unknown why some people develop this disease and others do not. Some experts believe it is due to an autoimmune disorder, in which an individual&#8217;s immune system reacts against part of your body. In vitiligo can be found in the blood very specific antibodies that act against the patient&#8217;s melanocytes.</p>
<p>Some families are more prone to autoimmune diseases than others. Another theory suggests that vitiligo is caused by nervous disorders, because the nervous system impairment is often associated with loss of skin pigmentation.</p>

<p>Permethrin cream 5% applied to the entire body at once has a similar efficacy to lindane 1%, but less neurological toxicity, particularly in children, so it is preferred. After their application across the body surface, leave on for 8 to 14 hours and removing the cream with a shower or bath. An application will often be enough.</p>
<p>Ivermectin, an antiparasitic oral, may also be effective in the treatment of scabies. A single dose of 200mcg per kilo of weight of ivermectin is as effective as or more than a single application of lindane 1%, but less than a single application of permethrin. We have seen that two doses of ivermectin achieved cure rates equivalent to a single application of permethrin.</p>
<p>The Center for Disease Control in the United States (CDC) recommends ivermectin (200mcg per kilo of weight orally in a single dose with another similar dose after two weeks) as an equivalent option to topical treatment with permethrin. This pattern has the advantage of easier use, avoiding the dermatitis associated with topical treatment and facilitates compliance. However, this drug has a greater risk of toxicity than permethrin. It is contraindicated in pregnant or lactating women and in children weighing less than 15 kg.</p>
